UTOPIA EVENT FORM
This test is offered by Xera Med Research specifically for attendees of Utopia event Sept 4-7.
Bring your official ID and health insurance card to testing (if you don't have insurance we will still administer the test at no cost to you).
You MUST submit signed release form (available below) allowing the results of your COVID-19 test to be released to the event organizer Tested Contained Retreats LLC, PRIOR TO TESTING.
